Membership Benefits

Membership Plan Cancellation Policy
Annual Membership (Paid in Full):
Membership fees are non-refundable
once benefits have been used. If no services have been utilized, a refund may be issued within 30 days of enrollment, minus any administrative fees.
Monthly Membership:
Membership requires a minimum 12-month commitment. If canceled before the 12-month term is complete, the patient will be responsible for:
- The full retail value of any services received, minus payments already made, or
- The remaining balance of the membership, whichever is less.
After the initial 12-month term, the plan becomes month-to-month and may be canceled with 30 days' notice.
General Terms:
- Membership fees are non-transferable and non-refundable beyond stated terms.
- Missed payments may result in suspension of benefits until the account is current.
- The office reserves the right to cancel membership due to non-payment or misuse of benefits.
